A female patient in her thirties, complained
of recurrent diabetic ketoacidosis, FOU, full investigations were done
without reaching to the actual cause of fever. In spite of many previous
normal abdominal ultrasonography, the last follow-up one revealed
enlarged right kidney, so CT scan was done and revealed the above photo.
However this picture confused us, the infectious nature of it was
strongly considered, so we considered it as a case of nephroma for which
the patient was given IV antibiotics for 2 weeks and then completed the
course by oral antibiotics for another 2 weeks.
She gave good response, the fever and constitutional symptoms subsided
also had controlled blood glucose levels with a decrease of the kidney
size
