Case 18

While I was hurrying to leave the hospital after a day of hard work, attracted me a side talk between my junior colleague about the hospitalized 30 year-old patient complaining of micturation-induced passing-out. When I listened to this, my will changed 100% towards seeing the patient. I rushed to detect a certain physical finding:

Do you know it? what is it?

For me I have detected it, also I have helped my colleagues to do the same. However, there were a lot of symptoms from which the patient suffered such as: easy fatigue, dyspnea on exertion and palpitations. These experienced symptoms persisted in spite of numerous medical consultations and investigations, all of them revealed no explanations.
