Case 6
This is the case as was presented to me by residents in some-clinical round:
A 16 years old patient presented with recurrent attacks of dyspnea which was severe enough to hospitalize the patient. Also there were recurrent attacks of convulsions.
There is history of similar attacks more than 10 years ago for which he was put , by a highly qualified neurologist ,on anti-epileptics for 3 years and since that time he was quite well up to one year when the symptoms recurred .
Examination: No physical findings .
The same case after polishing by me :
Islam is a 19 years old male ,complained 3 months ago of circumoral numbness which progressed to generalized body numbness with muscular spasms including carpopedal spasms.
His physical examination findings :
- Normal body built .
- No other abnormalities detected .
- -ve Chvostek's sign.
Investigations: - hypocalcaemia.
- hyperphosphatemia.
- hyperparathyroidism.
- other labs are normal.

What is the explanation for symptoms recurrence ?
