Juvenile S.L.E. presenting with portal Hypertension
-A 14 years-old female
-For two years she was and being complaining of
high-grade fever, associated with abdominal pain
-On presentation and by inquiry she also complains of
arthralgia and hair fall
-Clinically: generalized lymphadenopathy, hepatomegaly,
huge spleen
-My first impression that she has one of "collagenosis"
syndromes
-To cut this prolonged
suffering short:
- Labs:
-Bicytopenia; anemia & leucopenia
-moderately +ve
A.N.A.
-negative other autoimmune profile markers
-normal other laboratory values
-Abdominal
ultrasonography findings:
-hepato-splenomegaly
-porta-hepatis enlarged lymph nodes
-leino-renal ligament varices
-In view of this portal hypertension signs we should
confirm the existence of esophageal varices, which yielded
negative by endoscopy
-So, I consider this as to be
"Juvenile S.L.E. presenting with portal Hypertension"
-N.B., fever subsides firstly by non-specific treatment
-The plan is to start S.L.E.-specific treatment
promptly
